anyone else's quality score tank recently?

just started doing surveys this past month and worked my way up to 5 stars (somewhere in the 700s i think). i logged back on recently and i'm sitting at a 3.8 now, only one survey response rejected so far because i accidentally missed an attention check. does this happen often? just curious! hoping to get it back up soon
