Guanfacine *causing* insomnia; variations bw stimulants.

Tldr: guanfacine seems to cause insomnia and hyperactivity, while helping with emotion regulation. adderall causes insomnia too. med thoughts?

6yo boy, started trying meds 2 months ago. Main symptoms are hyperactivity, lack of social awareness, and emotional dysregulation with aggression.

Tried adderall; it did keep him a bit calmer and focused but even on 5mg INSTANT release given at 7am, he would be up 3 hours past bedtime building complex lego creations.

We switched to guanfacine ER 1mg at around 6pm and it helped him sleep for a few nights but wore off. So we went up to 2mg. Same, sleepiness wore off within a few days. But at this dose we got a new treat--high mood and hyperactivity to the degree I worried he was manic. We titrated down, but it seemed like focus and emotional regulation got bad quickly so (with psychiatrist approval of course!!) we decided to try again--maybe it was just coincidence?

Well, we got back up to 2mg a week ago and he's again extra hyper, extra sweet, and CANNOT SLEEP. i LOVE that he is able to control his anger better but he is so unfocused and so distracted and so hyper and so AWAKE and it is just untenable.

1) has anyone experienced this paradoxical response to guanfacine? would trying instant release or in the morning help?

2) has anyone found that their kid who had insomnia on one stimulant DID NOT on another stimulant? The psychiatrist said we should think about the Daytrana patch (ritalin, but gentler in a patch). I know from my own experience that different stims can have different side effects, but insomnia feels like a key issue-- like he is reacting to the very fact of being "stimulated" at all.
