Insulin Resistance increased in keto/low carb

So I'm kinda lost right now. I was diagnosed with insulin resistance 5 months ago, I've drastically changed my lifestyle ever since.

I went low carb (50g) and some periods keto (which I'm currently in), and I started resistance training.

Last time I tested HOMA it came back as 2.66.
I just got my last results and it came back as 3.08.

Basically my fasted glucose increased and my fasted insulin dropped by 0.7

I am not overweight, I've always been borderline underweight and I have to eat high fat in keto so I don't end up losing weight.

So my question is, am I dealing with Pathological Insulin Resistance and Physiological Insulin Resistance at the same time ? What am I supposed to do ?
