Help, prek school question

Sorry no one at my work is answering me, I feel so lost.

What is the difference between just a transition meeting and transition and annual? I understand the transition and annual is like a tri, but for the transition only do you just look at the report/assesment, and not touch the IEP? What if you didn’t assess? Thank you. This is in California.
